Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.

But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.

The Clinical Coordinator oversees the clinical aspects of the program, including intake and assessment, treatment and service plans, and coordination amongst other providers for service needs (i.e., detox, day treatment, psychiatric care, medical and recreational services etc).  The Clinical Coordinator will provide clinical supervision to a team of seven Case Managers. 


  • Provide clinical direction and leadership to the program; utilizing the Harm Reduction philosophy of the program 

  • Provide weekly clinical supervision to case managers and assuring that the concepts of Motivational Interviewing, Stages of Change and Harm Reduction are applied to the service delivery of consumers. 

  • Conduct Clinical team meetings and facilitate clinical case conferences 

  • Oversee mental health service provision, act as mental health liaison scheduling mental health appointments, track service provision, facilitate mental health case conferences 

  • Oversee development of treatment plans and implementation of service plans 

  • Oversee program documentation; i.e. progress notes & Face to Face contact 

  • Interview and assess potential program participants 

  • Supervise and conduct group case management and skills training 

  • Assess and evaluate resident functioning 

  • Document and report resident progress using social service software 

  • Assure regulatory compliance to the service delivery stipulated in the contract. 

  • Coordinate delivery of care with other agencies 

  • Works closely with Operations Manager to ensure client unit is maintained and accessible 

  • Work closely with Operations department to ensure client is paying rent monthly and rent arrears are addresses 

  • Performs other duties as assigned    


  • Licensed Master of Social Work 

  • Minimum 3 years of previous supervisory experience required 

  • Experience working with homeless and substance using populations. 

  • CPR and First Aid a plus 

  • Three to seven years related experience 

  • Exceptional oral and written communication skills 

  • Proficiency with Microsoft office suite 

  • Experience with foothold technology (A.W.A.R.D.S.) software preferred 

  • Preferred: have valid driver's license with driving record that is in good standing 

  • Must maintain a good driving record (as defined by the organization) and is a condition of employment
